Our Springfield Office is located at the corner of Old Jacksonville Road and Koke Mill Road, which is a quarter of a mile west of Veterans Parkway (Route 4) on Old Jacksonville Road. The building sits in the southeast corner of that intersection. Please view our Locations section for detailed maps.
You will need the patient’s Social Security Number as well as payment or proper payment arrangements at time of service. For more details, please refer to our self pay, worker’s compensation, or liability sections in our financial policy.
As a courtesy, we do submit insurance claims for our patients. It is important that we have your correct insurance information. We also accept cash, checks, debit cards, and VISA or Master Card. We can accept credit card payments by phone.
Our patient accounts staff will be happy to discuss your account and arrange a payment plan with you upon your arrival at our offices.

If you have paid OCI a copay or an amount due toward a deductible and, upon insurance processing, a self-pay credit remains, OCI will first apply that credit toward amounts due on your account at the time. If there are no patient amounts due, but you are scheduled in the future to come in, OCI will apply those credits to any copays you would normally pay at that time.

If a credit is left on your account and none of those situations apply to it, then OCI makes every effort to refund the patient as soon as they are aware of the credit due. In the event a check is not cashed, or a refund is not cleared, OCI will file the unclaimed property with the state, minus a $25 processing fee.
